Step 1: Go to Checkout

Login to your Shopify as usual, and go to the Settings button placed at the boom left corner of the page.

Then, choose the Checkout section from the menu.

Step 2: Find the Form options

When you're in the Checkout interface, scroll down until you see the Form options section.

Step 3: Edit the Form options

There are 4 fields to adjust, you can choose one option from each to have it on the checkout page:

• Full name
• Company name
• Address line 2 (apartment, unit, etc.)
• Shipping address phone number

Step 4: Save

After choosing the appropriate options, just click on the Save button at the top right of the page to apply the changes to the checkout page.
